Temporary Medical Parking Request Form

The Office of Parking & Transportation Services provides short-term parking accommodation for students and employees with temporary mobility-related disabilities (lasting less than 3 months).

  • Please complete the Temporary Medical Request Form. A verified letter/note from your physician must be attached to the application, that verifies the amount of time within 3 months that you will need temporary medical parking accommodations.
  • Once approved, you may use designated Temporary Medical Parking spaces.

If your condition lasts more than 3 months, please apply with your physician for a state-issued mobility-impaired license plate or hang tag through the Office of Motor Vehicles. ADA information and the ADA form can be found here, ADA Registration Form

• Parking in B Zones is not permitted with a Temporary Medical permit.
• Space is limited: Approval does not guarantee a Temporary Medical space will be available.
• If no Temporary Medical spaces are available, you must park in your assigned permit zone or use the Park & Geaux lot.
